چکیده

he remarkable effectiveness of aldosterone receptor antagnists in reducing the mortality risk in patients with dvanced heart failure (HF) (1) has led many to assume that he aldosterone receptor is directly involved in the process of eft ventricular (LV) remodeling, which seems to be a ritical mediator of the progressive syndrome of HF (2,3). his perception has been fed by preclinical evidence for ldosterone-mediated myocardial fibrosis and remodeling 4,5). But persuasive evidence has been lacking from an dequately controlled clinical trial to demonstrate that the avorable effect of this therapy could be attributed to its nti-remodeling effect rather than to another mechanism, uch as an antiarrhythmic response to potassium retention. urthermore, the interaction between aldosterone receptor lockade and the other neurohormonal inhibitors used in F has not been explored.

Aldosterone and end-organ damage.

PURPOSE OF REVIEW This review highlights recent clinical studies demonstrating the contribution of aldosterone to cardiovascular mortality, vascular dysfunction, and renal injury in the context of advances in our understanding of the molecular biology of aldosterone. RECENT FINDINGS Mineralocorticoid receptor antagonism reduces mortality in patients with congestive heart failure and following...

Randomized, double-blind, multicenter, placebo-controlled study evaluating the effect of aldosterone antagonism with eplerenone on ventricular remodeling in patients with mild-to-moderate heart failure and left ventricular systolic dysfunction.

BACKGROUND Aldosterone antagonism has been studied in patients with advanced heart failure (HF) and also in patients with post-myocardial infarction and left ventricular (LV) dysfunction with HF symptoms. Few data are available on effects of aldosterone antagonism in patients with mild-to-moderate HF. METHODS AND RESULTS In a multicenter, randomized, double-blind, placebo-controlled study in ...

Aldosterone antagonism in heart failure

Aldosterone, a neurohormone known to affect electrolytes, has recently been implicated as playing a major role in the progression of heart failure, particularly in patients with systolic dysfunction. Major clinical trials designed to analyze clinical outcomes using an aldosterone antagonist have been done in two groups with heart failure. The first was the Randomized Aldactone Evaluation Study,...

Mineralocorticoid receptor antagonism: therapeutic potential in acute heart failure syndromes.

Acute heart failure syndromes (AHFS) are a heterogeneous group of commonly encountered and difficult to manage clinical syndromes associated with high morbidity and mortality. Dyspnoea, pulmonary, and systemic congestion often characterize AHFS due to acutely elevated intracardiac filling pressures and fluid overload. Diuresis, respiratory support, vasodilator therapy, and gradual attenuation o...

Pleiotropic actions of aldosterone and the effects of eplerenone, a selective mineralocorticoid receptor antagonist.

Aldosterone plays an important role in the pathogenesis of cardiovascular and renal disease that is independent of angiotensin II. Mineralocorticoid receptors are expressed in nonepithelial tissues such as the heart and blood vessels. Although mineralocorticoid receptor antagonism reduces mortality in patients with congestive heart failure, the progestational and antiandrogenic side effects of ...
